**Job Title:** Dementia and Delirium Practitioner

**Estimated Salary:** £45,000 - £65,000 per annum

We are seeking a highly skilled Dementia and Delirium Practitioner to join our team at East Lancashire Hospitals NHS Trust.

The successful candidate will have extensive experience in caring for patients with dementia and delirium, and possess excellent clinical and leadership skills. They will be responsible for developing and implementing care plans, leading teams, and collaborating with other healthcare professionals.

Key Responsibilities:

  • Conducting comprehensive assessments and developing individualized care plans
  • Providing leadership and guidance to junior staff members
  • Collaborating with multidisciplinary teams to ensure seamless patient care
  • Participating in research and quality improvement initiatives to enhance patient outcomes

Requirements:

  • Registration with the NMC
  • Degree-level education in a relevant field or equivalent experience
  • Postgraduate qualifications in a related field
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
