We are looking for a skilled Holistic Care Practitioner to join our team. The successful candidate will have a strong background in nursing and a passion for delivering comprehensive, family-centered care.
In this role, you will work closely with children, parents, and healthcare professionals to develop and implement tailored care plans that prioritize the unique needs and goals of each family.
Key Responsibilities:
- Conduct comprehensive assessments of children's physical, emotional, and social needs.
- Develop and implement individualized care plans that address these needs and promote optimal health outcomes.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and coordination.
- Provide education and support to families, empowering them to take an active role in their child's care.
Requirements:
- Degree in Nursing (Bachelor's or Master's) from an accredited institution.
- Current registration as a nurse with the relevant governing body.
- At least 3 years of experience working with children and families in a similar role.
- Strong knowledge of child development, psychology, and nursing practices.
- Excellent communication, interpersonal, and problem-solving skills.
