Job Title: Social Worker – Young People with Disabilities Team

Job Summary: We are seeking a skilled Social Worker to join our Young People with Disabilities Team. As a Social Worker, you will be responsible for managing a caseload of children and young people with disabilities, providing assessment of their needs, advice, support, and the development of care plans and support packages.

Key Responsibilities:

  • Manage a caseload of children and young people with disabilities, providing assessment of their needs, advice, support, and the development of care plans and support packages.
  • Work with young people and their families to identify and address their needs, and to develop and implement care plans and support packages.
  • Collaborate with other agencies and professionals to ensure that the needs of young people and their families are met.
  • Provide emotional support and guidance to young people and their families, and to help them access services and resources.
  • Develop and maintain relationships with young people and their families, and to work with them to achieve their goals and outcomes.

Requirements:

  • Qualification in Social Work, such as a Diploma or degree in Social Work, CQSW, CSS or equivalent.
  • Registration with Social Work England as a registered Social Worker.
  • Demonstrable capability of practice in accordance with current professional standards, including the Professional Capability Framework (PCF) and the Knowledge & Skills Statement (KSS).
  • Able to use emotional intelligence, analytical skills, and to act with confidence in challenging social and family situations.
  • Able to use information technology and related systems, including word and social work case recording systems.
