About the Role

We are seeking an experienced Senior Case Manager to join our team in Hampshire, working with adults who require complex care support. As a key member of our multidisciplinary team, you will be responsible for assessing and planning individualized care packages, ensuring that each person receives tailored support to meet their unique needs.

Your expertise will help us provide high-quality services that promote independence, dignity, and well-being. You will work closely with individuals, families, and other professionals to develop and implement effective care plans, addressing physical, emotional, and social needs.

Key Responsibilities
  • Conduct comprehensive assessments to identify individual care needs and develop personalized care plans.
  • Collaborate with multidisciplinary teams to ensure seamless delivery of care services.
  • Work closely with individuals, families, and caregivers to understand their needs, preferences, and goals.
  • Develop and maintain relationships with local healthcare providers, community resources, and social services.
  • Monitor and evaluate care plans, making adjustments as needed to ensure optimal outcomes.
Requirements

To succeed in this role, you will need:

  • A Social Work qualification and registration with Social Work England (SWE).
  • Experience working with adults with complex needs, including mental health concerns, physical disabilities, and learning disabilities.
  • Solid understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
