About the Role:
We are seeking a skilled and compassionate Care Support Specialist to join our team at Torfaen County Borough Council. As a Care Support Specialist, you will play a vital role in providing high-quality care and support to individuals in their own homes, enabling them to live independently and safely.
Key Responsibilities:
- Deliver person-centered care and support to service users, promoting their independence and well-being.
- Work collaboratively with colleagues and other agencies to ensure seamless care delivery.
- Conduct thorough assessments and develop personalized care plans to meet the unique needs of each service user.
- Provide emotional support and guidance to service users, their families, and carers.
- Participate in ongoing training and professional development to enhance your skills and knowledge.
Requirements:
- Basic understanding of care in the community and experience of caring for people in a paid or voluntary capacity.
- Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of service users.
- Current driving license and use of transport is essential.
- Qualification to NVQ level II in Care or willingness to work towards Level 2 Diploma in Health & Social Care (QCF).
- Registration with Social Care Wales.
