About the Role

Lancashire & South Cumbria NHS Foundation Trust is seeking a skilled Health and Well-being Worker to join our Forensic Community Service. The successful candidate will work under the supervision of the Clinical Lead to support adults with mental disorders who have transitioned from secure mental health settings into the community.

Key Responsibilities

The Health and Well-being Worker will be responsible for:

  • Working independently with service users in various settings to promote recovery and reduce the likelihood of readmission
  • Supporting service users to identify their health and well-being needs and assist in planning, implementing, and evaluating activity within individual therapy treatment and recovery plans
  • Collaborating with the Multi-Disciplinary Team (MDT) to facilitate group and individual interventions
  • Developing rapport with service users and encouraging their interest and participation in specified therapeutic activities

Requirements

To be successful in this role, the Health and Well-being Worker will need to possess:

  • NVQ Level 2 or higher in Health and Social Care or equivalent
  • Ability to undertake Violence Reduction physical intervention training
  • Good communication skills, both written and verbal
  • Self-management skills and ability to work in a changing environment
