About the Job

We are seeking a highly skilled Community Practitioner to join our dynamic team in Gateshead, Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust.

As a Community Practitioner, you will play a vital role in delivering high-quality evidence-based interventions to individuals with complex mental health needs. Your expertise will be invaluable in supporting our service users to achieve their goals and improve their overall well-being.

Main Responsibilities

  • To assess and manage casework of service users with substantial and complex needs.
  • To deliver evidenced-based clinical and psychological interventions.
  • Coordinate treatment planning, reviews, and discharge planning.
  • To complete duty days supporting the Community Treatment Team when other clinicians are not available.
  • Work alongside other agencies to support service users with integrating into the community and addressing their biopsychosocial needs.

Requirements

  • Professional qualification and current professional registration as a First-Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Mentor Training.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Salary

The estimated salary for this role is £35,000 - £40,000 per annum, depending on experience.
