Job Summary

As a Reablement Practitioner, you will be responsible for conducting comprehensive assessments of need and developing supportive interventions as part of a multi-disciplinary team of Health and Social Care professionals.

Main Duties

Key responsibilities include:

  • Completing holistic assessments of need and developing individualized support plans
  • Working collaboratively with a multi-disciplinary team to deliver therapy-based interventions
  • Providing advice and information to service users to enable them to problem-solve and make informed decisions
  • Using a strengths-based approach to promote independence and well-being

Person Specification

Essential:

  • Previous experience in a Reablement, Rehabilitation, Health or Social Care setting

  • Knowledge of the care management process and social care legislation
