About the Role:

GLL is seeking a skilled and ambitious individual to fill the position of Health Referral Facilitator. As a key member of our team, you will play a crucial role in delivering our health and wellbeing objectives in our York Partnership.

Key Responsibilities:

  • Manage and oversee the successful delivery of the Healthwise program.
  • Establish and maintain strong relationships with GPs, health professionals, and local authorities.
  • Complete both practical and administrative elements of the Healthwise program.
  • Represent Healthwise in internal and external meetings.
  • Produce regular reports, including monthly, quarterly, and annual summaries.

Requirements:

  • Level 3 GP Referral qualification.
  • NVQ Level 2 Fitness Instructor certification.
  • REPS registered fitness professionals with excellent communication and interpersonal skills.
  • Experience in delivering and managing lifestyle interventions.
