Main Responsibilities

Provide a clinical pharmacy service to the gastroenterology ward on a rotational basis.

Support the provision and development of a holistic, specialist pharmaceutical care service to patients receiving high-risk, high-cost medication (advanced therapies, hepatitis anti-virals) delivered via homecare services.

Support services provided to outpatients and endoscopy and provide a clinical pharmacy service to the daycase unit.

Provide a ward-based clinical pharmacy service according to Trust agreed standards, including individual prescription review to optimize therapy, confirmation of the patient's medication history, advice on dosage, side-effects, cautions, and monitoring required, and offer duties.

Participate in ward rounds, clinical meetings, ward user group, or out-patient clinics as appropriate, to provide pharmaceutical advice to prescribers and other healthcare professionals.

Person Specification

Qualifications

Essential

  • Qualified to Masters Degree level (MPharm or equivalent).
  • Member of General Pharmaceutical Council (GPhC).
  • Post Graduate Diploma/MSc in Clinical Pharmacy or equivalent.

Experience

Essential

  • Clinical experience within gastroenterology/hepatology is highly desirable.
