Job Summary

This role is part of the Mental Health Review Team at Lancashire County Council, where you will support individuals in various settings who require additional assistance to live independently.

Key Responsibilities

  • Provide support to individuals in different settings, promoting their independence and well-being.
  • Work collaboratively with a range of people, including those with mental health needs, to deliver high-quality care and support.
  • Develop and maintain strong relationships with service users, their families, and other professionals to ensure comprehensive support.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to individual needs and goals.
  • Participate in on-the-job training and professional development opportunities to enhance skills and knowledge.

Requirements

  • Ability to work flexibly, including evenings and weekends, to meet the needs of service users.
  • Strong communication and interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
  • Capacity to work independently and as part of a team, prioritizing tasks and managing time effectively.
  • Commitment to delivering high-quality care and support, adhering to professional standards and guidelines.
  • Valid driving license and access to a car for business use (or willingness to consider alternative arrangements).
