Job Summary

As an Advanced Nurse Practitioner at The Highlands Practice, you will provide high-quality patient care, utilizing your expertise to diagnose, treat, and manage both acute and chronic conditions. Your role will involve working autonomously and as part of a multidisciplinary team to ensure patients receive comprehensive care.

Main Responsibilities

  • Undertake consultations with patients, using your clinical judgment to diagnose, treat, or refer patients as necessary.
  • Diagnose and manage acute and chronic conditions, integrating drug- and nondrug-based treatment methods into a management plan.
  • Make direct referrals to primary, secondary, and social services within locally agreed pathways, guidance, and protocols.
  • Work with patients and their carers to support compliance with and adherence to prescribed treatments.
  • Ensure care plans are in place, including emergency healthcare plans following discussion with the patient and family members.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within scope of practice.
  • Ensure complete and accurate documentation of each patient contact.
  • Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams to ensure patients' needs are met.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

Professional Requirements

  • Adhere to the NMC Code of Professional Conduct and work within the scope of professional practice.
  • Ensure professional practice adheres to organisational and NMC policies, procedures, and guidelines.
  • Maintain a professional manner and act as a positive role model to other members of the Primary Care team.
  • Maintain confidentiality with regard to information pertaining to patients and staff.
  • Ensure the service interfaces with all other departments in a professional and productive manner, providing an effective service to partner organisations and other service providers.
  • Where required, provide supervision and mentoring to other staff to help them develop their knowledge, skills, and abilities.
  • Deliver care according to NICE guidelines and evidence-based care.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.

This role is for Part/Full Time, and salary is dependent on experience.
