Medicines Management Specialist

To work alongside Clinical Pharmacists in supporting the Medicines Management service to the wards, ensuring patients receive the highest standards of care in medication management. Key responsibilities include medicines reconciliation, assessment of patients' own medications, checking medication for discharge, and counseling patients.
Main DutiesWe require a registered pharmacy technician to join our enthusiastic and motivated medicines management team. The post holder should be ready for a new challenge, demonstrate initiative in supporting service improvements, be a team player, and possess good problem-solving skills. The successful candidate will provide Medicines Management at ward level, be involved with medicines reconciliation, checking patients' own medications, initiating medicine supplies, providing information and advice to patients about their medications, and assisting with discharges.
