EHCP and Transitions Officer Role

We are seeking a highly skilled and experienced EHCP and Transitions Officer to join our dedicated team at a Special School for students with Profound and Multiple Learning Disabilities, severe sensory impairment, and complex medical needs.

Key Responsibilities:
  • Coordinate and administer the EHCP and review processes, ensuring compliance with statutory timelines and regulations.
  • Take minutes for Annual Review meetings and other required meetings, maintaining accurate records and ensuring confidentiality.
  • Liaise with Local Authorities, Social Care, and Health Professionals, as well as the school's multidisciplinary team, to ensure seamless transitions and effective support.
  • Coordinate transitions into the school for new students, including tours, assessments, and induction processes, to ensure a smooth and successful integration.
  • Develop external links with services such as work experience placements and other settings, to enhance the educational and personal development of students.
  • Host and lead training for families on transition-related topics, providing valuable support and guidance.
  • Track destination data and provide family support and signposting, ensuring that students receive the necessary support and resources.
  • Assist with general school office duties and administrative support for the headteacher, contributing to the overall efficiency and effectiveness of the school.
Requirements:
  • Knowledge of SEND and EHCP procedures, including the SEND code of practice and disability legislation.
  • Understanding of safeguarding children and vulnerable adults, with a commitment to maintaining confidentiality and upholding professional standards.
  • Familiarity with Data Protection principles and confidentiality protocols, ensuring the secure handling of sensitive information.
