About the Role
We are seeking a highly skilled and experienced nurse to join our team as a Clinical Health Specialist. The successful candidate will be responsible for improving the physical health care of Mental Health service users in accordance with directions and standards issued by the Physical Health Team.
Main Responsibilities
- Provide advice and support for a wide range of physical health monitoring and treatment services to Mental Health service users.
- Act as an advisor to all staff, helping to train and educate staff on all aspects of physical health prevention and promotion.
- Promote and facilitate the delivery of high-quality health improvement interventions, including smoking cessation and smoke-free clinics, healthy eating and physical activity initiatives.
- Work with other members of the Physical Health Team to lead on good practice around physical health care and infection prevention initiatives.
- Support the development, completion and analysis of physical health audits.
- Support the completion of a wide range of physical health checks for service users.
- Remain up-to-date with evidence-based practice guidance.
- Support the Physical Health Team to identify and develop links with health services to facilitate shared care pathways.
- Complete and maintain patient care records in accordance with Trust policy.
- Participate in the care and management of patients at risk of acquiring an infection and/or with an infection as and when required.
