About the Role
We are seeking a skilled and dedicated Nurse Practitioner to join our Community Nursing team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes and community settings.
Key Responsibilities
- Deliver evidence-based nursing care to patients with complex needs, working collaboratively with other healthcare professionals to ensure seamless care transitions.
- Conduct thorough assessments, develop and implement personalized care plans, and provide education and support to patients and their families.
- Work closely with multidisciplinary teams to coordinate care, manage referrals, and ensure timely access to specialist services.
- Participate in quality improvement initiatives, contribute to the development of policies and procedures, and engage in ongoing professional development to enhance skills and knowledge.
Requirements
- Registered Nurse with a valid NMC registration and a minimum of 2 years' experience in community nursing or a related field.

- Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage competing demands.
- Ability to work autonomously and as part of a team, with a flexible approach to working hours and locations.
