- Developing and implementing care plans that meet the unique needs of each client.
- Monitoring and evaluating the quality of care provided to ensure that it meets our high standards.
- Collaborating with other healthcare professionals to ensure that our clients receive comprehensive care.
- Providing regular feedback and coaching to care professionals to help them improve their skills and performance.
- Maintaining accurate records of client care and staff performance.
- Ensuring that our services are delivered in a safe and efficient manner.
- Developing and implementing policies and procedures to ensure that our services meet regulatory requirements.
Requirements
To be successful in this role, you will need:
- A strong background in care management, preferably in a domiciliary care setting.
- Excellent leadership and communication skills.
- A strong commitment to delivering high-quality care that meets the individual needs of each client.
- The ability to work collaboratively with other healthcare professionals to ensure that our clients receive comprehensive care.
- Strong problem-solving and analytical skills.
- Experience of developing and implementing care plans that meet the unique needs of each client.
- Experience of monitoring and evaluating the quality of care provided to ensure that it meets high standards.
