  • Developing and implementing care plans that meet the unique needs of each client.
  • Monitoring and evaluating the quality of care provided to ensure that it meets our high standards.
  • Collaborating with other healthcare professionals to ensure that our clients receive comprehensive care.
  • Providing regular feedback and coaching to care professionals to help them improve their skills and performance.
  • Maintaining accurate records of client care and staff performance.
  • Ensuring that our services are delivered in a safe and efficient manner.
  • Developing and implementing policies and procedures to ensure that our services meet regulatory requirements.

Requirements

To be successful in this role, you will need:

  • A strong background in care management, preferably in a domiciliary care setting.
  • Excellent leadership and communication skills.
  • A strong commitment to delivering high-quality care that meets the individual needs of each client.
  • The ability to work collaboratively with other healthcare professionals to ensure that our clients receive comprehensive care.
  • Strong problem-solving and analytical skills.
  • Experience of developing and implementing care plans that meet the unique needs of each client.
  • Experience of monitoring and evaluating the quality of care provided to ensure that it meets high standards.
