Nurse Practitioner for Patient Transition

The Talent Partnership seeks a highly skilled Registered Nurse to join our pre-assessment team as a Nurse Practitioner.

In this role, you will be responsible for conducting thorough patient assessments, developing individualized care plans, and working collaboratively with multidisciplinary teams to ensure seamless patient care and outcomes.

Key Responsibilities:
  • Conduct comprehensive patient assessments and develop tailored care plans
  • Collaborate with healthcare professionals to ensure effective patient care and outcomes
  • Maintain accurate and up-to-date patient records, adhering to clinical governance standards

Estimated Salary: £52,000 - £67,000 per annum, depending on experience

This full-time position offers 37.5 hours per week, with flexible working arrangements available. We are committed to supporting your professional development and offer regular training and mentoring opportunities.

About You:
  • A registered nurse with a minimum of 2 years' post-registration experience
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
