Community Health and Social Worker

We are seeking a dedicated and passionate Community Health and Social Worker to join our team in Gravesend.
This is an exciting opportunity to work in a hybrid model of office-based working and remote working, ensuring that you can meet the needs of our service while maintaining a healthy work-life balance.
About the Position
You will work with adults with a range of care and support needs, undertaking work of varying levels of complexity, including Care Act Assessments, Mental Capacity Act (MCA) Assessments, Court of Protection applications, and s42 safeguarding enquiries.
As part of our team, you will be responsible for building relationships with individuals, their families, and carers, and engaging with communities to empower them to find trusted help and support locally.
Our Support Offer
We offer a salary of £30,000 per annum, a generous annual leave allowance, and the local government pension scheme.
Additionally, you will have access to our social care academy, progression routes, and practice framework, as well as flexible working arrangements and regular professional supervision.
