About the Role

We are seeking a highly motivated and skilled Activity Coordinator to join our Inpatient Therapy Team at Chase Farm Hospital. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and focused outcomes.

Key Responsibilities
  • Contribute to assessing, planning, implementing, and evaluating patient care, working closely with the multidisciplinary team.
  • Facilitate a range of therapeutic groups and individual sessions, tailored to meet the needs of patients with acute mental health conditions.
  • Contribute to multidisciplinary team care planning, formulation, and risk assessment, ensuring comprehensive and individualized care plans.
Requirements
  • NVQ Level 3 in Care or equivalent training or experience.
  • Training in specific activities relevant to the client group, such as cooking, art, or craft.
  • Experience of facilitating groups, working with people from diverse backgrounds, and supporting individuals with long-term health conditions.
