About the Role:

We are seeking a dedicated and passionate Registered General Nurse (RGN) to join our team as a Community Nurse Case Manager. As a key member of our team, you will be responsible for providing high-quality care and support to clients in their own homes and communities.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop personalized care plans

Requirements:
