Key Responsibilities:
- Provide 24-hour care and support to the client, including managing their medical condition, meals, and hygiene.
- Deliver personalized care plans and health goals, enabling clients to live life to the full in their own homes.
- Maintain a positive relationship with the client, their family, friends, and those supporting their health and well-being.
- Respond to emergency situations and provide support with daily activities, such as preparing meals and accompanying the client to appointments.
