Family Support Coordinator

Salary: £28,817 - £40,464 per annum
Suffolk County Council's 0-19 Healthy Child Service is seeking a skilled Family Support Coordinator to join our team. As a key member of our service, you will be responsible for delivering targeted support to families in Suffolk, focusing on the social, emotional, physical, and intellectual wellbeing of children from pregnancy to 5 years.
Key Responsibilities- Develop and implement individualized support plans for families, taking into account their unique needs and circumstances
- Foster strong relationships with local community groups, healthcare providers, and other stakeholders to ensure a seamless service delivery
- Collaborate with colleagues to share best practices and contribute to ongoing service improvement
- Maintain accurate records and reports to track progress and outcomes, ensuring that data-driven decisions are made to inform service development
