Job Title: Outreach Family Support Worker

We are seeking a highly skilled and dedicated Outreach Family Support Worker to join our team. As an Outreach Family Support Worker, you will play a vital role in providing high-quality support to children and families in need.

Key Responsibilities:
  • Conduct early help assessments with families to identify their needs and develop support plans.
  • Chair Team Around the Family meetings to review progress and identify additional areas of support.
  • Provide direct work with children and young people to explore their views and worries.
  • Collaborate with multi-agency partners to ensure a whole family approach is taken to address identified needs.
  • Deliver targeted support to children and families to prevent the need for statutory services.
Requirements:
  • Experience of direct work with children and young people and their families.
  • Experience of effective collaborative and multi-agency working to support children and families.
  • Experience of delivering outreach and home-based support to children and families to address areas of identified need.
  • NVQ Level 3 in childcare and education or NND Health and Social Care qualification.
Working Hours:

35 hours per week.

DBS Check:

Enhanced DBS check required.
