Job Title: Occupational Therapist - Reablement Specialist

**Job Summary:**

We are seeking two experienced Occupational Therapists to join our team at Tripod Partners. As a Reablement Specialist, you will work with adults of all ages to assess and provide support for reablement, accommodation, equipment, and adaptations.

Key Responsibilities:

  • Conduct face-to-face reviews and assessments for adults of all ages
  • Provide support for reablement, accommodation, equipment, and adaptations
  • Triage to establish the possibility of using community resources instead of paid statutory services
  • Work collaboratively with clients, families, and healthcare professionals to achieve optimal outcomes

Requirements:

  • 2+ years of experience as an Occupational Therapist
  • Strong knowledge of reablement, accommodation, equipment, and adaptations
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
