Nurse Practitioner

We are seeking a skilled and compassionate Registered Nurse to join our team at our state-of-the-art aged care residence in Tamworth, NSW. As a Registered Nurse, you will play a vital role in providing high-quality care to our residents, promoting their health and wellbeing, and maintaining their dignity and independence.
Key Responsibilities:- Provide person-centered care to residents, assessing their needs and developing individualized care plans
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery
- Conduct regular assessments and reviews to ensure residents' needs are met
- Develop and implement care plans that promote residents' independence and autonomy
- Collaborate with other healthcare professionals to ensure residents receive comprehensive care
- Provide education and support to residents and their families on health and wellbeing topics
- Current unrestricted registration with AHPRA
- Demonstrated aged care experience preferred
- Excellent communication and clinical problem-solving skills
- Ability to work autonomously and as part of a team
- Understanding of and commitment to continuous quality improvement
- Experience in assessments and developing care plans
- Ability to create and update care plans to reflect developing needs of residents
- Understanding of Aged Care Quality Standards and Aged Care Funding Instrument
- Demonstrated organisational and time management skills
- Demonstrated well-developed computer and documentation skills
