About the Role

Cheshire and Wirral Partnership NHS Foundation Trust is seeking a skilled Palliative Care Specialist to join their team. This is an exciting opportunity for a qualified Registered General Nurse with a passion for delivering high-quality care to individuals with complex palliative care needs.

Job Summary

The successful candidate will provide holistic care and support to patients with malignant or non-malignant life-limiting conditions, working autonomously and as part of a multidisciplinary team. They will deliver care to patients registered with a West Cheshire GP in their own homes.

Key responsibilities include providing advice, education, and support to patients, their carers, and other healthcare professionals, promoting advanced care planning, managing complex end-of-life symptoms, and meeting the holistic care needs of patients.

Requirements
  • Registered General Nurse
  • Post-graduate qualification in palliative care or relevant subject
  • Experience of multi-professional working
  • Excellent communication and teaching skills
What We Offer

As a member of our team, you can expect a competitive salary of £38,890 - £44,036 per annum, depending on experience. In addition, we offer a range of benefits, including a relocation package of up to £8,000, which may be available to eligible candidates.
