Our team provides specialist clinical advice and care, supporting the delivery of all commissioned activities to adult patients within the North West Surrey area and delivering an integrated model of diabetes services.
Key responsibilities include:
* Conducting advanced clinical assessments and creating personalized management plans for patients with type 1 and complex type 2 diabetes
* Making autonomous, safe, and effective decisions to enhance patient health outcomes
* Leading and mentoring other healthcare professionals to enhance their skills and contribute to community and primary care development
* Driving the adoption of evidence-based clinical practices and developing specialist pathways within the service
We offer a competitive salary of £39,205 to £47,084 a year, inclusive of 5% High Cost Area allowance per annum pro rata.
