Reablement Team Lead Opportunity

The Greenwich Reablement Service is seeking an experienced Reablement Team Lead to join their team. As a Reablement Team Lead, you will be responsible for leading and developing the Reablement Service, working closely with the Integrated Discharge Team, Contact Assessment Team, and Joint Emergency Team.

Key Responsibilities:

  • Lead and develop the Reablement Service, ensuring high-quality interventions and outcomes for clients.
  • Work closely with the Integrated Discharge Team, Contact Assessment Team, and Joint Emergency Team to ensure seamless referrals and effective service delivery.
  • Supervise a team of Independent Living Assessors, ensuring they have the necessary skills and knowledge to complete Care Act 2014 assessments, MCA's, and Safeguarding concerns.
  • Chair Reablement Team Meetings, ensuring effective communication and collaboration among team members.

Requirements:

  • Proven experience in adult social care, preferably in a leadership or management role.
  • Strong knowledge of adult social care legislation, including the Care Act 2014 and MCA's.
  • Excellent communication and leadership skills, with the ability to motivate and inspire team members.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple deadlines.
