Patient Flow Coordinator

An exciting opportunity has arisen for a Patient Flow Coordinator to join our Complex Discharge Team at Barts Health NHS Trust. We are seeking an enthusiastic and motivated individual to work with our team, utilizing excellent organizational and interpersonal skills, good communication and IT skills, and the ability to work under pressure.
Main ResponsibilitiesThe successful candidate will support the Complex Discharge Team in facilitating patient flow and effective preparation of regular and detailed reports on medically optimized and delayed discharges for senior managers, consultants, and project board meetings. They will also undertake urgent discharge activities as required.

Person SpecificationThe ideal candidate will possess a good basic education to GCSE or equivalent level, with a strong understanding of challenges and pressures in a healthcare environment. Previous experience of working in a customer-focused or service area, discharge management, and social services liaison is essential. A commitment to a high standard of service and excellent communication and interpersonal skills are also required.
