Job Description:

  • Develop and implement individualised care plans in consultation with colleagues and stakeholders
  • Deliver person-centred care and support to individuals with complex needs
  • Participate in quality assurance activities to ensure high standards of care

Required Skills and Qualifications:

  • Relevant qualifications in health and social care (e.g., NVQ Level 3)
  • Experience working in a healthcare setting
  • Excellent communication and interpersonal skills
