Job Summary

We are seeking an experienced Advanced Practice Nurse to join our team. As a specialist in chronic disease management, you will play a crucial role in delivering high-quality care to our patients. Your expertise will be invaluable in managing complex conditions and promoting health and wellness through various clinics.

  • You will work closely with the management team to review clinical policies and procedures, ensuring that our practice remains at the forefront of healthcare innovation.
  • Your exceptional communication skills will enable you to provide travel health advice, vaccinations, and child immunizations, as well as family planning and women's health services.
Key Responsibilities
  • Managing chronic diseases, including diabetes, cardiovascular disease, and respiratory conditions
  • Implementing infection prevention and control measures to maintain a safe environment for our patients and staff
  • Obtaining blood samples and performing electrocardiograms (ECGs)
  • Minor and complex wound management, including leg ulcers
  • Promoting health and wellness through various clinics, such as healthy lifestyle workshops and educational sessions
Requirements
  • Essential:
    • Experience in a primary care environment, preferably as a practice nurse or community nurse
    • Registered on the NMC (Nursing and Midwifery Council)
