Clinical Practitioner

The University Hospitals Coventry and Warwickshire NHS Trust is seeking a highly skilled Clinical Practitioner to join the One Coventry Integrated Team. As a key member of the team, you will play a vital role in supporting the prehospital and consultant-led pathways, reducing unnecessary hospital admissions, and contributing to effective admission avoidance.
Main Responsibilities- Work as an autonomous practitioner to deliver high-quality patient care, utilizing advanced clinical skills and in-depth theoretical knowledge.
- Effectively manage a complex caseload of patients, providing specialist and evidence-based assessment and interventions to patients in both stable and acute phases of their condition.
- Utilize highly specialist knowledge and clinical reasoning to plan and provide clinical intervention in consultation with other colleagues in the multidisciplinary team.
- Signpost, refer, and support access to health, social, and third sector services to reduce inappropriate attendances and admissions at A&E and improve patient quality of life outcomes.
- Work collaboratively within the multidisciplinary team to plan and implement treatment for acute illness or complex chronic disease management as required.
- Participate in 7-day working rotas to ensure delivery of key services.
