About the Role

We are seeking a highly motivated and dedicated mental health professional to join our team as a Clinical Assessment Practitioner. As a key member of our multidisciplinary team, you will be responsible for assessing and managing referrals from GPs, external providers, and self-referrers.

The successful candidate will have a strong background in mental health, with experience in assessing and managing crisis situations. You will be able to work effectively in a fast-paced environment, prioritizing tasks and managing multiple referrals simultaneously.

Key Responsibilities
  • Assess and manage referrals from GPs, external providers, and self-referrers
  • Develop and implement individualized care plans in collaboration with patients, carers, and other healthcare professionals
  • Provide high-quality care and support to patients with severe mental health illnesses
  • Work effectively in a multidisciplinary team to achieve shared goals and objectives
  • Participate in the development and implementation of policies and procedures to improve patient care and outcomes
Person Specification
  • RMN, social worker, or occupational therapist with current relevant registration
  • Practice Assessor qualification
  • Excellent written and oral communication skills
  • Ability to assess clinical situations quickly and use initiative
  • Knowledge of counselling skills, cognitive skills, and behavioral skills
