Job Overview

We are seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will provide high-quality care to patients in their homes, promoting independence and improving health outcomes.

Main Responsibilities

  • Providing clinical care to patients, including wound management, palliative care, and symptom control.
  • Working collaboratively with the multidisciplinary team to support patients and their families.
  • Supporting patients to remain at home and avoid hospital admission.
  • Promoting self-care and independence.
