Transforming Patient Care in Primary Care

OneMK PCN is seeking an experienced Advanced Nurse Practitioner to join our multi-disciplinary team. As a key member of our team, you will play a vital role in transforming and modernizing pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries.

About the Role

You will have experience within a primary care setting and will be expected to provide high standards of patient care, inspiring those you work with. In addition to standard nursing duties, the ANP/ACP role is accountable for clinical governance of the practice and provides clinical leadership to ensure the safe, effective, and compliant delivery of services to all patients.

Key Responsibilities
  • Provide clinical leadership in the practice to ensure the safe and effective delivery of services
  • Accountable for clinical governance of the practice
  • Work with a great local team and have the support of the wider NHS services leadership team and our support centre teams
  • Be registered with current NMC/HCPC/GPhC registration
  • Have primary care experience as well as experience in minor illness management
