About the Role
We are seeking an experienced CNS to lead our Personalised Stratified Follow Up (PSFU) team at Barking, Havering and Redbridge University NHS Trust. The successful candidate will be responsible for implementing and managing the PSFU pathway for breast, colorectal, and prostate cancer patients.
Key Responsibilities
- Lead the implementation of PSFU for breast, colorectal, and prostate cancer patients
- Manage the Remote Monitoring Pathway for patients
- Develop and implement protocols for remote monitoring and supported self-management
- Monitor patient progress and escalate concerns as necessary
- Collaborate with multidisciplinary teams to provide personalized care and support
Requirements
- Experience working with the Personalised Care Agenda
- Formal presentation skills and ability to present complex information
- Project management experience or training
- Understanding of national and local health care developments and their impact on cancer care
- Ability to influence a range of stakeholders
