Key Responsibilities:- Coordinate care services, including assessment, planning, and implementation.
- Develop and maintain effective relationships with patients, families, and healthcare professionals.
- Collaborate with multidisciplinary teams to ensure efficient care delivery and optimal patient outcomes.
- Registered Nurse degree/diploma qualification.
- Current registration with the Nursing and Midwifery Council (NMC).
- Recent professional development and evidence of ongoing learning.
