Key Responsibilities:

  • Provide holistic, person-centred care to patients with mental health needs.
  • Work collaboratively with a multidisciplinary team to achieve personalised outcomes.
  • Support patients to live independently and reach their full potential.
  • Conduct assessments, develop care plans, and implement interventions as needed.
  • Collaborate with healthcare professionals to ensure seamless care transitions.

Requirements:

  • Current registration with the Nursing and Midwifery Council (NMC) as a Mental Health Nurse.
  • Previous experience in a community setting.
  • GCSEs in certain subjects, such as Maths, English, or Health and Social Care.
  • Fully enhanced DBS.
  • Car driver and valid UK license preferred.
