About the Job

SW Locums Ltd is recruiting for an experienced social worker to work in a specialist learning disabilities and autism service in Yeovil. The successful candidate will be responsible for carrying out strength-based assessments, person-centred support planning, and brokerage services to adult service users, carers, and their families.

Job Summary:

  • Full-time position available.
  • Monday to Friday schedule.
  • Salary up to £38.00 per hour.

Responsibilities:

  • To provide a social work service to an allocated caseload within the agreed performance management, quality assurance, and budgetary framework.
  • To act as a member of a team providing an effective, efficient, and personalised service to deliver key strategic aims.
  • To ensure people are fully supported to have the best possible opportunities and exercise choice and control to meet defined outcomes.

Requirements:

  • Social Work qualification with registration on the Social Work England register.
  • Relevant post-qualifying/vocational training courses and continuing professional development in line with the Professional Capabilities Framework (PCF).
  • Experience of working with carers and providing supervision and practice support to AYSE/Care Navigators.
  • Knowledge of relevant legislation, including the Mental Health Act 1983, Mental Capacity Act 2005, Disability Discrimination Act 1995, Care Act 2014, and Safeguarding.
