Job Summary

To provide high-quality nursing care to children and young people with palliative care needs, working as part of the Integrated Care Team in the hospice, home, or hospital. The post holder will be required to work a variety of shifts, as the hospice provides care services 24 hours a day, 365 days a year.

Key Responsibilities

Provide high-quality nursing care for children and young people with palliative care needs.
Promote a person-centred care approach within the multi-disciplinary team.
Contribute to care plans and assessments to ensure the child/young person receives safe and effective care from hospice services.
Liaise with the lead for mental capacity assessments and ensure these are completed as required.
Ensure care is planned and delivered in accordance with the Care Quality Commission Inspectorate standards of quality and safety.
Be fully conversant with and contribute to all clinical guidelines, policies, and procedures of Haven House.
Communicate complex and sensitive information effectively to service users, care staff, and other members of the team.
Further develop clinical skills and knowledge and maintain own competency for NMC portfolio.
Work with the Nurse Practice Educator in delivering training and further support junior colleagues with responsibility towards healthcare support workers.
Ensure that safeguarding is embedded in practice and concerns are escalated at the earliest opportunity.
Contribute to effective medicines management in accordance with Haven House policies.
Develop and maintain clinical skills and knowledge necessary to provide holistic evidence-based nursing care.
Act as an advocate for the child and family.
Ensure that the child/young person's nutritional needs are met whilst at Haven House.
Have a knowledge of the commonly used drugs in paediatric palliative care and undertake the relevant drug administration competencies.
Have a knowledge of safeguarding policies and procedures.
Use Haven House's Electronic Patient Record System to ensure data is entered in an accurate and timely fashion, in accordance with policy and procedure.
Demonstrate a professional approach to work and act in accordance with the NMC Code.

Working Collaboratively

Constructively identify issues with care strategy, policies, and procedures in the interests of the organisation and users of the service.
Contribute in the evaluation of services as required, ensuring that children, young people, and family's views are represented.
Be aware of allocated resources and ensure effective time management in own role.

Clinical Governance

Participate in audit, service user feedback collection, and research as required.
Report clinical and non-clinical incidents in accordance with policy.

Education

Identify own learning needs and ensure professional development is maintained and contributes to the appraisal system.
Attend all mandatory training as required by the service.
Mentor pre-registration nursing students, providing feedback to students' assessor.
Contribute to overall staff and service development by updating the evidence base for the team re clinical practice.
Education and training of parents/carers to meet the healthcare needs of their child.
Education and training of junior staff and students to meet the healthcare needs of service users.
Comply with Haven House's arrangements for continuing professional development.

Professional

Adhere to Haven House policies and NMC Code of standards of conduct, performance, and ethics for nurses and midwives.
Keep clear and accurate records.
Maintain confidentiality at all times in conjunction with Data Protection and Caldicott guidance.

Other Duties

Promote and live the Haven House values of Compassionate, Professional, Respectful, and Safe.
Work with colleagues across the hospice as part of the Haven House One Team approach.
Conduct yourself in line with the Haven House culture and behaviours at all times, acting as an ambassador for the hospice.
Contribute towards a culture of continuous improvement to help drive our service to Outstanding.
Offer flexible support for the work of the organisation, with a flexible approach to new duties and responsibilities as needed to support our vision, mission, and values.
