Social Care Specialist

Job Summary:
We are seeking a skilled and compassionate Social Worker to join our Integrated Community Mental Health Team. As a key member of our team, you will work closely with clients, carers, and other professionals to provide holistic assessments, commission packages of care, and ensure the best possible outcomes for our service users.
Key Responsibilities:
- Undertake community care assessments, working collaboratively with clients, carers, and other professionals to identify needs and develop suitable packages of care.
- Design and organize packages of care, ensuring adequate resources are available and that financial assessments are conducted in conjunction with carers and statutory agencies.
- Liaise with local authority services, partner agencies, and informal service users and carers to meet needs in the most efficient and appropriate way.
- Monitor and review service user needs and provision on a regular basis, providing direct support and services to individuals and families.
- Collaborate with colleagues within the Health and Social Care Teams, participating in supervision, staff development, and ERDP.
- Maintain accurate case records, providing information for performance indicators, unfulfilled procedures, and statutory duties in line with national and local policy practices and procedures.
Requirements:
- Experience of working in a Health/Social Care setting, with a focus on community care and case management.
- Demonstrable understanding of main issues facing Community Care, with relevant professional qualifications in Social Work.
- Effective communication skills, with the ability to manage change, work to deadlines, and under pressure.
- Self-confidence, self-motivation, and ability to manage own workload, with competent use of IT equipment and software.
- Ability to negotiate and liaise in relation to care management, with a commitment to confidentiality and a willingness to accept direction/delegation.
Working Arrangements:
This is a temporary post for 1 year, working 28 hours per week. You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service.
