At CRA GROUP LIMITED, we are seeking an experienced Mental Health Social Worker to join our team in Adult Social Care. As a key member of our team, you will be responsible for coordinating care for complex cases under the Care Programme Approach (CPA).
Key Responsibilities:
- Developing co-produced care plans with service users
- Risk assessment and management
- Liaison with inpatient units
- Accurate record keeping and report writing
Requirements:
- Must have experience of working with adults with psychosis and complex needs
- Experience of Care Act assessments and reviews
- Good knowledge of relevant legislation, policy, and procedure, including Safeguarding, Mental Health Act, Care Act, and Mental Capacity Act
