Job Title: Mental Health Care Coordinator

We are seeking a highly organized and compassionate Mental Health Care Coordinator to join our team at Central & North West London NHS Foundation Trust. About the Role:

The successful candidate will be responsible for coordinating the delivery of physical health care services, working closely with multidisciplinary teams to ensure seamless delivery of care, and developing effective strategies for improving patient outcomes.

The salary for this position is approximately £45,000 - £55,000 per annum, depending on experience.

Key Responsibilities:
  • Coordinate the delivery of physical health care services to patients with severe and enduring mental health problems.
  • Work closely with multidisciplinary teams to ensure seamless delivery of care.
  • Develop effective strategies for improving patient outcomes.
