Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, offering them hope, kindness, and a brighter future.

About the Role

This is a unique opportunity to make a significant difference in the lives of individuals impacted by substance use and homelessness. You will work closely with our outreach team to deliver healthcare interventions, provide guidance, and support to service users, and contribute to the evaluation of evidence-based practices to end rough sleeping and reduce homelessness.

Key Responsibilities
  • Provide clinical healthcare services to individuals experiencing homelessness, including assessments, treatment, and support.
  • Work collaboratively with our outreach team to develop and implement individualized care plans.
  • Contribute to the evaluation of evidence-based practices to inform policy and service development.
  • Participate in training and professional development opportunities to enhance your skills and knowledge.
Requirements

To be successful in this role, you will need:

  • A degree in nursing or a related field.
  • Experience working with vulnerable populations, particularly those experiencing homelessness.
  • Strong clinical skills and knowledge of healthcare interventions.
  • Excellent communication and interpersonal skills.
  • A commitment to delivering high-quality services and contributing to the evaluation of evidence-based practices.
What We Offer

We offer a competitive salary range of £43,814.50 - £48,183.48 (pro rata for part-time hours), a fixed-term contract until February 2026, and the opportunity to work with a compassionate and dedicated team at Change Grow Live.
