About the PositionIn this crucial role, you will facilitate effective communication between healthcare providers, families, and individuals, ensuring holistic care delivery. With strong interpersonal skills and adaptability, you will navigate diverse situations and make informed decisions that drive positive outcomes.
Our County-wide initiative focuses on early intervention, risk assessment, and tailored support. You will collaborate with cross-functional teams to develop comprehensive care plans, enhancing the quality of life for those we serve.
Essential Qualifications and Skills- Post-graduate education in nursing or related field (e.g., BSN, MSN).
- At least 12 months' experience in a community setting or comparable role.
- Advanced training in non-medical prescribing.
- Certified knowledge of safeguarding, child protection, and multi-agency panels.
- A annual salary range: £34,500 - £44,000.
- A variety of continuing education opportunities.
- A highly collaborative team atmosphere.
