Healthcare Portfolio Manager

About the Role
We are seeking an experienced and skilled Team Manager to join our North Lincolnshire Community Directorate. The successful candidate will be responsible for the day-to-day operational management of a portfolio of services, ensuring that patients, carers, and stakeholders can navigate through care pathways with ease and have their needs met within recovery-focused evidence-based provision.
Main Responsibilities
The post holder will be responsible for:
- Ensuring that services meet their targets in line with Service Specifications
- Liaising directly with GPs and other services to ensure effective responses to clinical issues
- Offering expert opinions to staff on clinical matters such as risk and appropriateness for treatment
- Working with the Service Manager to ensure that services meet their targets in line with Service Specifications
- Liaising with acute directorate, community directorate, and NHS Talking Therapies to ensure good working relations and a smooth pathway of care for patients
- Ensuring team activity supports efficient use of resources in line with an allocated budget and ensures team activity is in line with required performance parameters and quality standards
- Supporting Business Planning within the Care Group in line with their sphere of expertise and responsibilities
