About the Role

As a Care Professional in our Home First Neighbourhood Team, you will play a crucial role in transforming hospital discharge services to meet the needs of Oxfordshire residents in their own homes.

You will work closely with reablement providers and the voluntary sector to support individuals regain independence through a strength-based approach. This includes goal planning for short-term care after hospital discharge or to prevent hospital admission.

We are looking for a skilled professional who can use activity analysis, assistive technology, and minor adaptations to reduce formal care. Your expertise in moving and handling skills is also essential for this role.

Key Responsibilities:

  • Support individuals to regain independence through a strength-based approach
  • Assist with goal planning for short-term care after hospital discharge or to prevent hospital admission
  • Use your expertise in activity analysis, assistive technology, and minor adaptations to reduce formal care
  • Apply moving and handling skills as part of your role

What We Offer:

  • A competitive salary of £35,745 - £42,403 per annum
  • Up to 33 days holiday plus bank holidays (option to buy additional days)
  • Membership of the Local Government pension scheme (up to 19.9% employer contribution)
  • Relocation package of up to £8,000 (conditions apply)

About You

We are seeking a self-leader with a passion for making a difference in adult social care. As a Care Professional, you must be committed to continuous learning, supported with development opportunities.

You should be driven to find creative solutions and work collaboratively with individuals, rather than for them. Additionally, you should be compassionate, respectful of diverse backgrounds, and advocate for equality.

A valid driver's licence holder with access to a vehicle is also required for this role.
