Homecare Specialist

About the Role

At Cheshire East Council, we are seeking a highly skilled Homecare Specialist to join our Community Reablement Team.

This is an exciting opportunity to work in a person-centred manner, supporting individuals to achieve maximum independence with personal care and daily living skills.

You will play a vital role in promoting health and wellbeing by providing short-term interventions to encourage individuals to regain their independence.

Working closely with healthcare professionals and other agencies, you will assess individuals' needs and develop tailored support plans to help them reach their full potential.

As a Homecare Specialist, you will be responsible for:

• Developing and implementing personalized support plans">
• Providing training and education to individuals and their families">
• Collaborating with healthcare professionals and other agencies to ensure seamless service delivery">

Salary and Benefits

We offer a competitive salary of £24,294 - £25,979 per annum, depending on experience.
