We are seeking a skilled and compassionate Adult Social Worker to join our Locality Team. As a key member of our team, you will work closely with adults who require support with complex needs, including physical disabilities, mental health issues, and learning disabilities.
Your primary focus will be on assessing individual needs, developing personalized care plans, and facilitating access to services that promote independence, dignity, and well-being. You will work in accordance with the Care Act 2014 and other relevant legislation to ensure that our service users receive the highest level of care.
Key Responsibilities- Conduct thorough assessments of adults referred to the locality team, identifying their needs, risks, and strengths. Develop and implement care plans that promote well-being and independence.
- Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005. Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individual rights.
- Handle urgent and complex cases with sensitivity and efficiency, providing crisis intervention and support as needed.
- Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards.
